The Federal Employees Health Benefits (FEHB) Program offers numerous health benefits plans and coverage options. Choosing a health plan and coverage option is an important decision. To help you make an informed choice, each FEHB plan (Piedmont Community HealthCare) makes available a Summary of Benefits and Coverage (SBC) about each of its health coverage options, online and in paper. The SBC summarizes important information in a standard format to help you compare plans and options.

If you are enrolled in this plan, you are entitled to the benefits described in the brochure. If you are enrolled in Self and Family coverage, each eligible family member is also entitled to these benefits. You do not have a right to benefits that were available before January 1, 2012, unless those benefits are shown in this brochure. A link for the 2013 benefits brochure is shown below.
OPM negotiates benefits and rates with each plan annually. These benefit changes are effective January 1, 2012, and the specific changes for the current benefit year are summarized on page 9. Rates are shown at the end of this brochure.
